CFP - Special Issue of JNRT: Resistance to recombinant human erythropoietin therapy in haemodialysis patients

Anaemia is a common complication in haemodialysis patients, resulting in a significant mortality and morbidity in these patients. This anaemia is associated to a decreased bone marrow production of erythrocyte, due to the inability of the failing kidneys to secrete erythropoietin (EPO). The introduction of recombinant human EPO (rhEPO) therapy led to a significant reduction in anemia and improved patients’ quality of life. However, there is a marked variability in the sensitivity to rhEPO, with up to 10-fold variability in dose requirements to achieve correction of anemia. Approximately 5-10% of the patients show a marked resistance to rhEPO therapy.

This variability in the response to rhEPO therapy remains relatively unexplained. rhEPO resistance is associated with several condition. However exclusion of these factors does not eliminate the marked variability in the sensitivity to rhEPO therapy.

CFP - Special Issue of JNRT: Renal Transplantation in South Asia

Renal transplantation has been regularly and successfully been performed in south Asia, specifically India since the 1970’s. At present, about 10,000 transplants are being performed in India per year.

The region became quite popular in the mid 1980’s as a centre for renal transplant ‘medical tourism’, ignominiously called the ‘Bombay transplants’ leading on to the govt. Of India intervening and introducing the ‘human organ transplant act’ in mid 1990’s,making such transplants illegal and also setting up a framework for a cadaver transplant networking program.

Pakistan also recently introduced a bill on similar lines. The region has certain issues specific and unique, because of social, cultural and economic factors. There are a number of NGOs and institutes promoting cadaver renal transplantation, which is now being performed in many centers in India since mid 1990’s in the absence of a registry, centralized networking or any state support.

Certain other clinical problems are post transplant infections, need for individualizing induction strategies, sensitizing the state and the public at large towards organ donation.
A wealth of experience is available in the region to address these issues and suggest solutions.
